If you are a former patient (i.e., haven't been seen in over 6 months) we will do our best to work with you. If we are not able to schedule you due to conflicts then we will refer you to other therapist who may have the availability. Step 1: In consideration of your time and determining the right fit for treatment, a ten-minute phone consultation is recommended before the approval of any therapeutic services. If you get our voice mail leave your name, number and a brief message and we will do our best to follow-up within 1 -2 business days.
Step 2: Please click on the link below to go to our secure client portal to setup your profile and complete the registration process (intake, assessments). From there you will be able to request your first and any future appointments. Once you request an appointment(s) they must first be confirmed via email/text to secure your date & time. NOTE: It is best that you complete the registration process on a computer to ensure all data is saved correctly.
Step 3 (optional): Your therapist may be required to provide a Professional Disclosure Statement (PDS) during the intake process. If you would like to confirm this and/or obtain one prior to therapy, please contact our office for further assistance.

- Suspected child abuse or dependent adult or elder abuse, for which we are required by law to report this to the appropriate authorities immediately.
- If a client is threatening serious bodily harm to another person/s, we must notify the police and inform the intended victim.
- If a client intends to harm him/herself, we will make every effort to enlist cooperation to ensure their safety. If they do not cooperate, we will take further measures without the clients permission provided to us by the law.
